Gigabyte GA-78LMT-USB3 USB ports randomly shuts off and back on

I've had this board for 3 years and it's giving me a headache with the USB ports shutting off and back on randomly when I'm using the desktop.

AMD FX-6300 3.5GHz 6-Core CPU
12GB DDR3 (1x 4GB, 1x 8GB)
480GB SanDisk Ultra SSD
1TB Seagate SSHD
1TB Seagate HDD
650W Antec PSU
LG Blu-Ray RE
LG DVD-RW
nVidia GeForce GT 730 (will upgrade to a better one)
Sound Blaster Surround 5.1 Pro USB External Sound card
Razer Naga Hex keyboard (RGB) and mouse
Logitech RumblePad 2 (for Battlefield 2 and other games)
ASUS BT400
D-Link Wireless AC1200 USB 3.0
Ultra ATX Case from the early 2000's
Xbox One S Controller (for Fortnite and other games)
7-port USB hub that can be powered using an optional power supply
Samsung SyncMaster 215BW (DVI-D) and ASUS VK222H (HDMI)

Is it because of a setting in either the BIOS or the hardware properties in Windows that's causing this? I'm going to purchase another board of the same brand with more PCI-e slots for an internal sound card and Wireless-AC + Bluetooth PCI-e cards. I also checked to make sure it wasn't shorting out on the case and the solder joints aren't remotely close to it.
