Mac OS 8.6 ISO is corrupt
The ISO was improperly ripped, grabbing only the HFS partition, and not including the APM partition map.
FWIW, file should be able to detect a "pure" Mac ISO. Unfortunately, it isn't aware of hybrids, but those should "just work," and are archived easily due to Apple being smart about how APM/HFS are included in hybrid images.
